Acute pulmonary toxicity associated with high-dose vinorelbine and mitomycin C.

چکیده
Vinorelbine (VNB), 5-nor-anhydrovinblastine, is a new semisynthetic vinca alkaloid with a broad spectrum of antineoplastic activity both in vitro [1] and in vivo. Promising therapeutic results have recently been reported in different tumour types, including lung, breast, head and neck cancer, and Hodgkin's disease [2]. Data from clinical trials indicate that VNB is safe and well tolerated, with neutropenia being the most common and dose-limiting toxicity [3]. Nonhematologic adverse reactions are mostly mild or moderate, and because of the selective activity for mitotic microtubules [4], drugassociated neurotoxicity occurs less often than with other commonly used, natural vinca alkaloids. Respiratory reactions have only rarely been observed in patients treated with VNB [3]. We report here a case of acute, severe pulmonary toxicity in a patient receiving high-dose VNB and mitomycin C (MMC) for treatment of advanced breast cancer. A toxicity analysis of 87 patients treated with similar VNB dose regimens, that has been performed in view of this case, revealed a total of 16 episodes of this unusual, most likely allergic, adverse reaction.

A Phase II Study of Vinorelbine, Mitomycin C and Cisplatin Chemotherapy for Advanced Non-Small Cell Lung Cancer
BACKGROUND This prospective phase II trial was performed to determine the efficacy and toxicity of mitomycin C, vinorelbine and cisplatin combination chemotherapy for patients with previously untreated stage IIIB or IV non-small cell lung cancer (NSCLC). METHODS Between January 1999 and April 2001, 30 patients with chemotherapy-naive stage IIIB or IV NSCLC were entered into this study. Mitomy...

OBJECTIVE The purpose of this pilot study was to evaluate the feasibility and toxicity of concurrent chemotherapy with vinorelbine and mitomycin C in combination with accelerated radiotherapy (RT) in patients with locally advanced cancer of the head and neck. PATIENTS AND METHODS Between January 2003 and March 2004, 15 patients with T4/N2-3 squamous cell carcinoma (12/15) and with N3 cervical...

gesting that mitomycin C/vinorelbine should be further tested in combination with RT in HNSCC. However, these results should be considered with caution. Indeed, as opposed to the meta-analysis database which provides reliable and mature results based on large numbers, the trial from Wilkowski et al. has very small numbers, was not randomised, and had a very short follow-up. In 2005, the key que...
متن کاملAge and Pharmacokinetics of Antineoplastic Agents
As the geriatric population is growing, it is increasingly important to be familiar with chemotherapy for the elderly. Age-related changes in pharmacokinetics are documented for doxorubicin, etoposide, ifosfamide, daunorubicin, mitomycin, cisplatin and methotrexate. The hematological toxicity of most standard-dose chemotherapy is not affected by age in patients with normal organic functions and...
متن کاملNephrotoxicity of high and conventional dosing regimens of colistin: A randomized clinical trial
BACKGROUND: Nephrotoxicity has been a major long-standing concern about colistin. This study was designed to compare nephrotoxicity of high dose and conventional dose of colistin. METHODS: A randomized open-labeled clinical trial on 40 patients with multi-drug resistant gram negative infections was designed. Patients were allocated into two equal-size groups receiving high and conventional dose...
